Resolution: Turns out this was an artifact of having both combat action bar options enabled, but not "showing" that setting to Mafia so it could adjust. Viewing my settings has resolved the issue fully. The combat history in my install of KOL Mafia is nuts. Every combat round ends with a repeat...
